Please be aware that we are a small office and our staff dedicates its care and attention to our patient experience in the office as much as possible. For this reason, many phone calls go to voicemail initially. All calls received during regular business hours will be returned within one business day. (All existing patient matters are answered more quickly through the Patient Portal.)

Medical Records Request
To request a copy of your medical records, please contact our office through the patient portal or email our team with your full name, date of birth, and the specific records requested. Requests are processed in accordance with applicable privacy laws and may require identity verification. You may also fax a request or signed release form to 512-309-5437.
